Administration

The Ashtabula County Board of Developmental Disabilities' Service and Support Administrators, or SSA's, coordinate and connect eligible individuals with developmental disabilities to the supports they need to help them achieve their goals. SSA's assist individuals to coordinate services among different agencies. SSA's also work with families, guardians and natural supports to provide whatever services an individual may need. All services provided are person centered and assist individuals to imagine the life they want to live, believe they can create it and achieve their dreams as participating members in their community.

Service and Support Administrators are the primary point of coordination, which includes:

 

Working with individuals and their families to administer assessments and collect documentation to determine eligibility for services

Coordinate assessments to find out what is important to individuals and what is working and not working in their life.

Develop an Individual Support Plan that outlines what is important to/for individuals

Assist individuals in selecting a provider of their choice

Connects individuals to community resources they need to be successful

Provide ongoing coordination and monitoring to ensure services are delivered as outlined in the Individualized Support Plan developed by the individual and their team

Who can make a referral?

 

Referrals can be made by parents, friends, relatives, teachers, doctors, hospitals, social service and human service agencies or anyone who believes that a person residing in Ashtabula County may benefit from and qualify for services from Ashtabula County Board of DD.

What is the criteria for eligibility?

Eligibility criteria varies by age. Below is a brief overview by age.

3 - 5 Years Old

6 - 15 Years Old

16 Years and Older

Eligibility is based on assessment results showing a significant developmental delay in adaptive behavior, communication, physical development, learning and/or social-emotional functioning.

Eligibility is determined through:

     Verification of the presence of a qualifying disability; and

     Implementation of the Children's Ohio Eligibility Determination Instrument (COEDI) that verifies substantial limitations in at least 3 of 6 life areas.

Eligibility is determined through:

     Verification of the presence of a qualifying disability; and

     Implementation of the Ohio Eligibility Determination Instrument (OEDI) that verifies substantial limitations in at least 3 of 7 life areas for youth and adults.
