Ashtabula County Board of Developmental Disabilities

Female, living in Austinburg, is looking for an HPC provider to take her to the community for activities and errands. She uses a wheelchair and needs transportation in a wheelchair van. She currently has a few hours of staffing during the week but needs more hours. She would need staffing on Monday – Friday Mornings, 7:00 a.m.- 8:30 a.m., and Monday, Wednesday, and Friday, 2:15 p.m.- 5 p.m. She would also like to have hours during the day to volunteer.
