Ashtabula County Board of Developmental Disabilities

Male, living in Ashtabula (with plans to move to Lake Co.), is looking for 24 hour HPC services with a portion OSOC. He can take up to 35 hours a week of alone time. Services include HPC and HPC MILES AND TRIPS IN A MODIFIED VEHICLE. He needs help with running errands, grocery shopping, meal prep, activities in and out of the home and chores. He utilizes a motorized wheelchair but will need assistance when transferring between his wheelchair and other platforms.
