Who can make a referral?
In order to receive services from the Ashtabula County Board of Developmental Disabilities, the person needing support must live in Ashtabula County and meet the eligibility criteria established by the Ohio Department of Developmental Disabilities.
Intake & Eligibility
Referrals can be made by parents, friends, relatives, teachers, doctors, hospitals, social service and human service agencies or anyone who believes that a person residing in Ashtabula County may benefit from and qualify for services from Ashtabula County Board of DD.
What is the criteria for eligibility?
Eligibility criteria varies by age. Below is a brief overview by age.
Birth - 3 Years Old
3 - 5 Years Old
6 - 15 Years Old
16 Years and Older
Eligibility is based on diagnosis, delay based on the score of the Bayley Scales of Infant & Toddler Development 4, or Informed Clinical Opinion as well as determining need for services based on a family assessment of daily routines.
Eligibility is based on assessment results showing a significant developmental delay in adaptive behavior, communication, physical development, learning and/or social-emotional functioning.
Eligibility is determined through:
Verification of the presence of a qualifying disability; and
Implementation of the Children's Ohio Eligibility Determination Instrument (COEDI) that verifies substantial limitations in at least 3 of 6 life areas.
Eligibility is determined through:
Verification of the presence of a qualifying disability; and
Implementation of the Ohio Eligibility Determination Instrument (OEDI) that verifies substantial limitations in at least 3 of 7 life areas for youth and adults.
