Male, living in Andover, is looking for HPC and HPC transportation services. He is looking for 25 hours a week (decided between Mom and the provider). He struggles to communicate or express himself. He has PICA, he elopes, likes to climb to high places and has self-injurious behaviors sometimes. He needs to be spoken to in a calm voice, encouraged to use supportive technology, redirected, engaged in fun activities such as jumping on the trampoline, playing outside and going on car rides. He needs complete assistance with taking medications, support to engage in outings, help scheduling and accompanying to medical and dental appointments. He needs assistance to complete routine hygiene with reminders and supports during bathing and close by to help if needed while toileting. He requires visual supervision at all times inside and close/constant outside the home and in the community. It is important for him to watch his tablet, to go on car rides, play outside, go swimming, to be active, to have staff speak calmly and reassuringly to him and prepare his favorite foods. He needs consistency in his routine and ensures that reminders and support are given. He is looking for a friend to be adventurous with him. Mom prefers the provider to be athletic and he likes to be active outside a lot and wants staff to jump and play with him outdoors.
