Male, living in Ashtabula, is looking for HPC services 24 hours a day, with a portion OSOC. He has 20 hours a week of alone time. Services will include HPC and HPC miles and trips in a modified vehicle. HPC duties will include: errands, grocery shopping, meal prep, household chores and home/community activities. He utilizes a motorized wheelchair and will need assistance with transferring between his wheelchair and other platforms.
*He plans to move to Lake County.
