13 year old male, living in Jefferson, is looking for HPC and HPC Transportation services. The schedule would be (during the school year, starting 8/08/2025) Monday-Friday 5:30am-8am, then 2pm-7pm. On Saturdays and Sundays, the hours would be 7am-7pm. OISP will be written to reflect school days off. He struggles to communicate and express himself. He has PICA, he elopes and likes to climb to high places. Sometimes, there will be self-injurious behaviors. The provider can support him by speaking in a calm voice, encouraging the use of supportive technology, redirection and engaging in fun activities with him (jumping on the trampoline, playing outside and going on car rides). He needs complete assistance with taking medication, support to engage in activities, help scheduling and attending appointments, completing hygiene with reminders and support during bathing. He needs staff close by while he’s using the bathroom in case he needs help. He likes to have alone time in his room (with check-ins). It is important for him to watch his tablet, go on car rides, play outside, go swimming, to be active, have staff speak calmly and reassuringly and prepare his favorite foods. It is important for staff to be close by at all times, to offer redirections and to be consistent with his routine. Reminders and support need to be given. He needs help with managing his medications and appointments. He is looking for a friend to be adventurous with him. Mom prefers the provider to be athletic as he is young and likes to be active outside a lot and wants staff to jump and play with him. The start date is ASAP.
