2025-88 Level One Waiver, HPC and NMT Needed GENEVA
Male, living in Geneva, is looking for staff every other Friday from 2pm-6pm and some weekends (Saturday and/or Sunday) for community outings, errands, etc… He has a history of engaging in inappropriate texts with ex-girlfriends or inappropriate phone use. He is open about these texts. He will report them or give honest information if asked. Supports talk through appropriate phone use with him. He lives by himself and is in need of a HPC provider who supports him with scheduled hours and drop in services. He sees his mom regularly and check in with her, SSA or staff daily. He is able to be alone as long as he’s comfortable and discusses this with the team. He is safe with general supervision in the community and depending on the environment, he may not need staff at all. His family is very involved and often takes him to events. Important to and for him are family and friends, maintaining a work environment with his peers and establishing friendships, saving money for things that he wants to buy and having support to do the things he wants to do. Some other things important to and for him are to have help from Mom to manage his finances, making the bigger decisions in life that he may want or need help with, going to day program and transportation to his day program. He likes to be able to socialize with his friends and be offered help with daily chores. The support of his family and staff are important to him in order to help him and offer advice, redirection and reminders as needed. STAFF MUST BE A NON-SMOKER.
