Male, living in Andover, is looking for HPC services. OISP will be written to reflect student days off during school year, so that if there is no school, and mom works staff will be scheduled full day. 

Known & Likely Risks Struggles to communicate or express himself. He has PICA, eloping, likes to climb to high places, SIB sometimes. 

Here’s How A Provider Can Support Me: speaking in a calm voice, encouraging use of supportive technology, redirection, engage in fun activities with him such as jumping on the trampoline, playing outside, going on car rides. Needs complete assistance taking medication, support to engage in outings, help scheduling and accompanying to medical and dental appointments. He needs assistance to complete hygiene routine with reminders and support during bathing, and close by to help if needed help while toileting. 

Visual supervision at all times inside, and close and constant outside the home and in the community.

Important TO and FOR Me: It is important for youth to watch tablet, go on car rides, play outside, go swimming, be able to be active, have staff to speak calm and reassuring to him, and prepare his favorite foods. It is important for youth to have staff close by at all times, offer re-direction, be consistent in routine, ensure reminders and support are given, help with managing medication, and appointments. Stephen is looking for a friend to be adventurous with him.

Desired Start Date: ASAP

Are there any exclusions for the provider (smoker, animal allergies, etc…)? Mom prefers provider be athletic, as youth is young and likes to be active outside a lot, and wants staff to jump and play with him outdoors.
