Ashtabula County Board of Developmental Disabilities

Female, living in Geneva with a Level One Waiver, is looking for NMT, HPC and HPC Transportation.  She can receive 10 hours per week of HPC to take her out in the community. She receives 5600 miles in HPC transportation to take her to and from church as well as other community activities as she requests. She has community integrated employment NMT and receives 6 trips per week. Her work address is 7590 Auburn Rd, Painesville. She works 2/3 days a week from 3-7:30 varying days. She does not require the use of a wheelchair.
