Ashtabula County Board of Developmental Disabilities

 

Female, living in Ashtabula, has IO Waiver funding and is in need of HPC services and HPC transportation services. The schedule is as follows: OSOC daily from 11pm-7am, HPC services Friday-Sunday 12pm-8pm, Monday-Wednesday 9am-8pm and Thursday 8am-8pm. This is a set schedule. She attends college and needs the chosen provider to take her to and from college a couple times throughout the week. She has remote services in place 33 hours a week. She needs help attending appointments and getting groceries. She needs to be encouraged to do chores and help with reminders and refills for her medications. She has 6,000 miles per span. She does not require the use of a wheelchair.
