Ashtabula County Board of Developmental Disabilities

 

27 year old male is seeking a waiver home. He is verbal, but can be hard to understand at times due to his enunciation of words. He can be shy and tends to only speak to people that he knows well and feels comfortable with. He can be silly and joke around. He enjoys watching cartoons on YouTube, playing video games (Super Monkey Balls Two) and being on the computer. He enjoys taking pictures or videos on a tablet. He likes vacuuming and spraying spray bottles. He can be sexually inappropriate with exercise balls and bean bag chairs. He could elope at times especially when in a new environment. Rarely, he will throw items when he is frustrated. When driving in the car, he may try to open the door or attempt to throw items out of the window. These behaviors typically occur when he desires attention, is fixated on an item, or wants to escape a task. Individual needs physical assistance with bathing, brushing his teeth, wiping after a bowel movement until clean, hair care, and nail care. He requires total assistance for diabetes management including monitoring his carb intake, checking his glucose levels, and monitoring for Catatonia and insulin administration.
A provider would need to be able to deliver all ISP services as determined at the ISP meeting. The provider would need to have experience with individuals with behaviors such as possible elopement and sexual behavior with inflated sports/exercise balls. The Provider would need to be Medication Administration Cert. 3 to assist in diabetes management, insulin administration.
TRANSPORTATION NEEDS: non modified vehicle.
DIAGNOSES:  moderate intellectual disability, autism, catatonic disorder, anxiety, type 1 diabetes (insulin dependent diabetes mellitus-brittle) and OCD.
HOUSING: Individual currently resides in his Family home with his parents in Madison but is looking to move out.
