2026-22 IO Waiver, HPC + HPC Transportation Needed JEFFERSON
14 year old male with an IO Waiver, living in Jefferson, needs HPC + HPC transportation 25 hours weekly. It is a varied schedule.
Struggles to communicate or express himself. He has PICA, eloping, likes to climb to high places, SIB sometimes. Mom reports that this has lessened a lot + he is doing well. It is best for staff to speak to him in a calm voice, encouraging use of supportive technology, redirection, and engaging in fun activities with him such as jumping on the trampoline, playing outside + going on car rides. Needs complete assistance taking medication, support to engage in outings, help scheduling and accompanying to medical and dental appointments. He needs assistance to complete hygiene routine with reminders and support during bathing, and close by to help if needed while toileting. Visual supervision at all times inside, and close and constant outside the home and in the community. He likes to watch his tablet, go on car rides, play outside, go swimming, be able to be active, and prepare his favorite foods. It is important for youth to have staff close by at all times, offer re-direction, be consistent in routine, ensure reminders and support are given, help with managing medication, and appointments. He is looking for a friend to be adventurous with him. Mom prefers staff to be more athletic to be able to play with him.
