Male living in Saybrook seeking 17 hours of HPC around his day program and natural supports. He has natural support overnight. This person is ambulatory. This individual benefits from reminders and assistance with many ADLs. Individual requires assistance with meal planning, shopping, cooking, and organizing/cleaning his areas of the home, reminders and assistance with hygiene, reminders for twice daily medications. Transport to occasional medical appointments. Assistance scheduling appointments as a back up to natural supports. He benefits from support to communicate. He requires assistance in the event of an emergency. This individual enjoys bowling, bingo, and meals out at restaurants. He participates in a Saturday bowling league from September to April. Supervision level is general in the home and close and constant in the community. Ideally two hours in the evening after day program, longer shift on Saturday, and a few hours on Sunday. Exact times to be determined between the family and the provider.
