A 24 year old female, living in Ashtabula, is starting college on Tuesdays and needs the provider to be able to take her to and from Mentor early in the morning. Transportation is 6,000 miles per span. She has remote services in place. Individual needs help with attending appointments and grocery shopping. She needs encouragement to do chores and help with reminders as well as refills for her medication. It is a set schedule; HPC daily 9am-8pm + 10pm-11pm; OSOC daily from 11pm-7am
