23 year old male, living in Jefferson, is looking for HPC + HPC Transportation services 15 hours per week. It will be a varied schedule and services will be needed as activities occur. Transportation will be discussed at the time of interview. He needs support to ensure he remains seizure-free and that, if he does have a seizure, the correct protocol will be followed. He needs support with ADLs such as oral hygiene, bathing and nail/hair/skin care. He requires verbal prompts to complete most of these. He needs support when communicating his wants and needs, as well as total support when shopping and handling his funds. He needs physical help with meal prep and general housekeeping duties. He will need total support when in the community at activities and requires close + constant supervision to ensure his safety when on outings.
