He uses a wheelchair and requires hands-on assistance with transfers in and out of his wheelchair. He needs physical assistance with all activities of daily living (ADLs), including personal care and daily household tasks. He is unable to independently administer his medications and requires certified staff to administer them safely.
Due to his mobility limitations, he requires access to a wheelchair-accessible van for transportation to medical appointments, community outings, and other activities. 1500 miles per span.
