41 year old female, living in Jefferson, is looking for HPC and HPC transportation 16 hours a week (typically Mondays for 8 hours and Tuesdays for 8 hours). She uses a wheelchair and needs a modified vehicle for transportation. She currently lives in her family home. She needs assistance during meal times, moving about her home and the community. She is able to verbally communicate her wants and needs.
